GP Management Plan

What is a GP Management Plan?
A GP Management Plan (GPMP) is a formal treatment plan developed between you and your GP if you’re living with a chronic or complex condition—something that has persisted for six months or longer, such as diabetes, arthritis, heart disease, or persistent injury. This plan helps manage your ongoing healthcare needs and may allow you to access subsidised sessions with allied health professionals, including exercise physiologists like the team at Allied Movement.
Who is eligible for a GP Management Plan?
You may be eligible for a GP Management Plan if:

You have a chronic medical condition (for example: pain, diabetes, cardiovascular disease, or arthritis) that’s been present for at least six months.

Your GP agrees that a structured approach to managing your care will benefit you and can outline goals and actions in your plan.

Your GP can also prepare Team Care Arrangements (TCA), especially if you need ongoing multidisciplinary care from multiple health professionals (such as a physiotherapist and an exercise physiologist together). From July 1, 2025, these are streamlined into a single GP Chronic Condition Management Plan (GPCCMP).
The Process: Getting Your Plan and Exercise Physiology Referral

Book an appointment with your GP
Let your doctor know you want to discuss your eligibility for a GP Management Plan for exercise physiology. Take any relevant health information and be prepared to discuss your past treatments and current symptoms. -
Assessment and Plan Development
Your GP will determine eligibility and, if appropriate, write a plan that includes:-
Your chronic conditions or health needs,

Treatment and management goals,

Recommended health services (like exercise physiology),

Frequency and type of services (your plan can include up to five Medicare-subsidised allied health sessions of any kind per calendar year).

GP Referral to Allied Movement
Make sure your GP lists exercise physiology in the plan and issues a referral to Allied Movement. Your GP can use an official referral form that you can then share with Allied Movement at your first appointment. -
Book Your Session with Allied Movement
Contact Allied Movement to book your initial assessment. Bring a copy of your referral and any medical information your GP provides. At your first session, your exercise physiologist will conduct a comprehensive assessment, discuss your goals, and develop an individualised plan to support your recovery or health improvement.
How Many Sessions and What’s Covered?

You can access up to five Medicare-subsidised allied health sessions per calendar year under a GP Management Plan. These sessions can be all with an exercise physiologist or split among different providers (e.g., EP, physiotherapist, podiatrist).

There may be a small gap fee; check with Allied Movement for their current rates and bulk-billing policies.
